Charity Care

It is the policy of Confluence Health to provide equal access to its facilities and services without unlawful discrimination on the basis of race, color, national origin, disability, age, sex, sexual orientation, gender identity or expression, creed, religion, marital status, veteran or military status, inability to pay for medically necessary evidence-based care or for urgent or emergent services, insurance coverage under Medicare/Medicaid/CHIP, or any other status protected by law. Confluence Health offers a Charity Care Program to patients based on family size and federal poverty guidelines.
Please mail or fax your completed application to Confluence Health (including all documentation). Be sure to keep a copy for yourself.

If you need assistance filling out the charity care application, please call Patient Services at (509) 436-4020 or visit any of the following locations below. To submit your complete application in person, please visit one of our following Patient Services areas:

  • Confluence Health I Omak Campus: 916 Koala Dr. Omak, WA 98841. Hours: Monday–Friday, 7 a.m. to 4 p.m. Phone: (509) 826-1800.
