Who We Are
Your case manager is a professional who is committed to help you achieve your best health possible and provide guidance for your wellness needs. Confluence Health offers free case management services for anyone living with HIV or AIDS in Chelan, Douglas, Grant and/or Okanogan Counties. These services are intended to help you maintain stability and connect with any needed support or resources.
What We Do
HIV case management offers support in many life domains and helps to connect you to needed services and resources in your area. Some of the support services offered are:

Education and prevention

Ensuring basic needs are met

Access to transportation

Insurance coverage
-
Access to rental assistance resources

Access to mental health services
-
Access to peer support
-
Support with appointment and medication adherence
To be eligible for HIV/AIDS case management services, a client must:

Have a current medical diagnosis of HIV or AIDS

Not be receiving concurrent HIV/AIDS case management services through another program

Reside in the state of Washington

Provide identification and income verification
-
Obtain insurance coverage (staff can assist with this)
If you meet the criteria and wish to participate, you will be assigned a consistent case manager to work with.

In Coordination with End AIDS Washington (EAW) our goals are:

50% reduction in the rate of new HIV diagnosis by 2020

Increase viral load suppression for people diagnosed with HIV to at least 80% by 2020

Reduction in related age-adjusted mortality rates among persons with HIV by 25% by 2020

Reduction in racial, ethnic, and other disparities in health outcomes for people living with and at risk of HIV

Improve quality of life for people living with HIV
